We had this issue in HRSSA. The FYI details would show up in the notification in the OAF page, but not in the email notifications. We ended up having to get a patch from Oracle. I realize that's not your particular issue, but thought it helpful to share.
the problem is solved by setting the personalization at Site level

when notifications are processed by the mailer; the below context is used:
* "SYSADMIN" user
* "System Administrator" responsibility
Please make sure that your personalization is well visible for the above context.
