The fix for the Substitution Cancelled issue missed 4.1.1. The workaround is to remove any login script reference in Tools > Preferences > Database.
If you want anything the login script would normally setup in the session, just run it manually after connecting. For a prior discussion of this, see...

If you have a startup script preference try removing it, the preference may have been set by a preference imported from an earlier release.
There was a bug logged just before release.

This seems to have fixed the problem.
Ironic that I just posted an issue a few days back indicating that my report output was no longer formatting correctly and the resolution was to create a connection startup script which I have never used until I started using 4.1.1 .
Now I have to consider creating a snippet so that I can quickly access all of the report formatting preferences or I keep the start up script and convert all of my scripts to use a :value reference instead of a &value reference. I'm starting to think that it would be nice to have report formatting options in the OSD preferences but then allowing it to be overridden by a startup script should one be defined. This would make these issues easier to deal with until a fix comes along.
Alternately, procrastination until 4.1.2 is an option
