They probably entered their password wrong too many times and their account got blocked.
It has been set to NONE in the EBS frontend
How do you know it has been set via the form and not automatically due to password failures or similar?

Is your users knows the apps password?
If yes they can change the FND_USER from through API's i hope, because many times will come across this type issues
Without our knowledge they will add responsibilities from back-end.
