Successfully installed 12.2.0 and upgraded to 12.2.6 using the following documentation:

No errors or issues encountered during the upgrade to 12.2.6.
After the first cutover, the following 4 files did not make it to webtier/bin so could not login to the apps after cutover:
Copy missing dlls from E:\oracle\vis122\fs1\FMW_Home\webtier\bin or E:\oracle\vis122\fs2\FMW_Home_obsoleted_2017_02_05_11_53_09\webtier\bin

After replacing these files and running autoconfig, could login to EBS again.
The fs_clone step is not copying the APPL_TOP at all and fails to start adadminsvrctl.cmd
Has anyone successfully completed an adop patching session on 12.2.6 on Windows or have any suggestions on how to resolve? I have attempted the fs_clone 3 times with no change.
