Is this happening in all browsers or just IE? Please clear the cache on the browser and the server.
Try deleting the 'Temporary Internet Files' in IE and clear the cache on the server:

This is happening on all browsers , IE, FLOCK, MOZILLA, CHROME etc. we have cleared the cache and and deleted temporary files as suggested but the problem still persist.
is there any setting in the application that must be done to fix this issue?
I have same issue whem applied RPC5 on R12.1.3 issue and i resolved by the below steps
Backup and delete the stylesheets in $OA_HTML/cabo/styles/cache and bounce the opmn services
check the below documents and thread
