You could run the following command to check the database creation date which could be the ideal installation date
SQL>select NAME ,CREATED from v$database

You can check the CREATED column from select * from v$database; to know the instnace creation date
For more info from installed log files if available.
For Upgrade you can check as below
select * from ad_bugs where bug_number like '9239090'; -- here you can see CREATIN_DATE colum
9239090 is R12.1.3 Patchset
May I have the functional tool/form to have the said info?
There is no tool required as this can be received from sql query on respective standard table.
May you please share the summary of respective tables with respective columns names? where we may get related/required info.
Actually there is need to know that from when the cst is using EBS & when it was upgraded.

You can check the following
select NAME ,CREATED from v$database
select BUG_NUMBER,CREATION_DATE from ad_bugs where BUG_NUMBER='<patchnumber>'
<patchnumber> >> If it is not 12.2 version of EBS, then you can provide the upgrade patch number of the version which you want to know.
