Please ensure you have a clean oraInventory with write permissions for the database user the clone is being executed from.

Thank you mdtaylor for the quick response. I found out the reason for the error; but forgot to update this thread.
I checked the oraInventory, /etc/oraInst.loc, etc. and all seemed good. Dug deeper into the logs and found the cause of the error in ohclone.log (excerpt below). The error was caused by our Linux box running out of swap memory. Apparently our Linux Admins had just rebuilt a new box and the configuration of memory settings was off. After the swap memory issue was fixed, the error was gone. But thank you again for your help. It is always appreciated.
