Its better to run fs_clone if you had applied patches in downtime mode.

1 - Do I need to start admin server in patch file system?

2 - Do I need to run preclone on both run and file system in admin node ?

3 - Is it ok to run preclone in run file system alone?

2- Only on run file system

3- Only on run file system you should run preclone.

Make sure your adop session is complete. Run preclone on both the apps tier and the database tier. 

adop -status -details - check it is good to go.
