It would be very difficult to troubleshoot as there is not even a single log file to look at.
I would suggest to follow the required notes as it is so that you are sure you have not missed anything. You could also log an SR so that you get a support engineer who has more expertise in this area
I followed the same but the documents tells "Ensure the WebLogic Administration Server is running from both run and Patch file system on the primary application tier node.". It's not recommeded stop all the application services except admin server.
We used to stop our application while adding node . But that statement itself is confusing to be honest

So we have to stop all the application services except admin server before start add node configuration. Can you confirm on that one more time I would follow the same.
We always stopped our applications while adding node . Since you are not getting any breakthrough you can try that as well
