What you want in the format you want is not possible.
The categories can be assigned only after creating an item.
Do you have any naming convention?
e.g. IT items begin with the word IT, Medical items being with the word MED- etc.
If you have a convention like that, you could write a personalization that looks at the beginning of the item and then checks the user's responsibilities to ensure that he should create an item like that.
Controlling access to modify is a little more easier.
You can simply make the block non-updateable based on the responsibilities of the person (or his /her HR dept) and the categories of the item.
