Why can't I create an online form from my custom record?

I am trying to create a survey through Netsuite using these steps https://netsuite.custhelp.com/app/answers/detail/a_id/9415 on the sandbox, but I do not have the option to create an online form from the custom record:


I do have the option to create a new online form through Set Up> Marketing> Online Customer Forms> New, but doing it this way does not give me the fields I created for my custom record as an option.


How do I get the ability to create a new online form from my custom record like the directions state?
