Online Customer Form - County/State Field

When configuring an Online Customer Form, the County/State field does not have the options for 'Search' or 'Select' - they are greyed out:

I presume there may be some issue with this as in the main UI you need to select the Country before you can get a valid list of Counties to select from.

How can we make this work on an Online Customer Form?

Even without the use of a drop down list, I can't get the County field to successfully populate from the form, either using the County Name, or the County Short Name (which is what we had to use to import all the County names into the addresses).
