How to revert Online Form Country Field?

Since the latest update all our online customer forms are broken.

Users cannot submit any form because the Country field has changed from a simple Select element, to an annoying ugly search pop-up, which doesn't even pop up in most of my custom form templates.

How can I revert this back to using the normal Select field that has worked flawlessly for many many years??
