How to create Invoice Without Sales Order?

Hi, Gurus!

I need to be able to create an invoice directly without an SO. However, when I do so through

Transactions>Sales>Create SO

the credit is to "Purchases Returned Not Credited". Why does it not show the revenue account that I have selected in item record's "accounting tab"?

Alternatively, when will a revenue account be hit?
