Hierarchical Custom Record New Null

Hello all,

I have started noticing this behaviour on a few of my accounts now. When I create a custom record and set Hierarchy check box to True this tab is displayed on all the forms.

I would assume that it is a tab for it sub records, but I cannot change the name of the tab and why is the button New Null
