how do I add a transaction form link to a button

Hi,

I am trying to link a transaction form from invoice. I add the button using workflow, but when I click the button I wanted to open a credit memo transaction form.

this is the workflow I did

now what will the action for opening the transaction form? I tried go to record,go to report the new form does not show up.

can you please help?
