How can I join a replacement PO to an existing SO?

Here are the knowns

  •  The SO was not changed.
  • The Item on the PO was changed.
  • The SO & PO are now separated. 
  • The SO has no knowledge of the new item.  
  • Need to know how to join the SO & PO back together.  with old item on SO and new item on PO.
