Multiple Initiating Forms for the Same Process

Can a review process that has several tiers of review have an initiating form at each level?
Example: Multitier review process. Employee can submit a form to start the process that is sent to a supervisor. In addition, the supervisor can initiate the same process at the supervisor level with a different form. Review and initiation at the supervisor level are separate.
I have a customer who has designed a process like this for a service request. The additional forms do not work so it is not clear if this is not supported or there is an error in implementation. I don't believe this is a good practice in any case. But if it can be done, are there guidelines the customer should follow?
