Unable to create incidents from advance transaction control

I have created a transaction model which is retrieving the transactions which are at risk. When I deploy the control with the same model, it is not creating any incidents out of it. Could you please help to understand what might have gone wrong?

Am not sure whether am doing anything wrong with the execution procedure, It would be a great help if you can provide some training documents related to Advance Controls.
