Build a FRS report across prior eleven periods and Years once user enters particular period and year

Hi Experts,

Our client has a requirement to build a FRS report showing prior eleven months and corresponding year of prior eleven months once the user entered the year and period in the user prompts.  For an example, Lets say user inputs FY18 as the year and Apr as the period in the user prompt. So report should show the following template. How can i design a report like this in FRS? Is it possible? Highly appreciate your help on this matter.
