Not getting rows included option in Supplemental Data Management form

Hi,

Am not getting data rows adding option in Supplemental data management form. Here I duplicated the Jan form template and deployed for Feb month. If I use the Jan template and deploy for Feb period there am getting rows adding option. Please view the attached screenshots in doc. Can you let me know what I missed out?
