User Login report - 6/29 data columns mixing up

Something has changed.  I modified the user login report to include some custom fields.  It's been running fine through 6/28 ... and  starting 6/29 I notice my last columns have been mixed up BUT more importantly, the "notes' column has "garbage" in it and the not the actual notes (yyyy-mm-ddThh:mm:ss).  Wish I would've caught it sooner.  Anyone else?

I have learned that if we re-download the file, it can fix itself.  So ... what happens when the "last report" is selected for download?  For some reports it takes time before I can open the file?  Is there something wrong with my Excel ... do I need to contact Microsoft?  Or does Oracle do something with the file to gets to my computer?
