I'm looking for a way to create a report that lists transactions imported by user via Smart View

Our internal business partner is asking us for creating a report for SmartView transaction tracking. 

This report should include such info as:

• Who uploaded transactions via SmartView

• Identify of Transaction

• When transaction was uploaded
