Job requisition report not pulling all job requisitions

Summary:

I created a job requisition report to show all open requisitions in a month. I need it to show any that are currently open plus those that were open and filled in the same month. The report I created is randomly missing one or two requisitions each month and it is not the same requisition. Does someone have a job requisition report they would be willing to share or help me understand why my report is randomly missing requisitions?

We have to report all job requisitions by month to the State so it doesn't matter if they were open months ago and still open or if they were open and closed in the same month. I need a report that will show, by month the number of all requisitions.
