Is there a report which can track the provisioned/deprovisioned and deleted roles for a user?

We are unable to find any report that helps us track the audit history of a user account regarding when the roles were/are assigned and when the roles were/are removed from the user.

We have a report (User History Report) which shows the roles which have been last assigned to a user and also the details of the person who assigned that with the date and time of assignment. However, this report is valid only if the user is currently having the role. We are unable to trace any information related to any deleted roles belonging to that user, thereby making the details incomplete.
