Docusign - CPA - Template - Not showing the CPA Amount / Date etc.

Hi,

We are testing the docusign integration for CPA documents using the demo.docusign.com account. When we send the document for signature the PDF document is not showing the CPA Amount / Date / Revision etc.

Any idea where this PDF template is stored and how to get this populated in the CPA doc?
