Performance Document Changes Audit Report

Hello,

We have a need to audit our Performance Documents to understand who all made changes on a performance document. Each time a rating is changed we want to generate a row on the report with the rating, the date it was updated and who made the update. This is especially important as we have HDL updates at times. In addition, if we could see who moved a Performance Document forward or backward in the process and the date of that change, it would also be very helpful.

I can't find anything in OTBI with that level of specificity, but does anyone have recommendations for tables/views in BI where we could gather that information? If OTBI is possible, please let me know.
