How to fetch particular attributes at Header Level in PFI Report from Customer Accounts section?

Client want to fetch only these attributes at header level in PFI Report:-

1. Company Name
2. Address
3. City
4. State/Province
5. Zip/Postal Code
6. Country
7. Phone
8. First Name & Last Name.

Based on the details he chooses in Customer Details while creating Sales Order, above attributes should fetch in PFI report.
