User and Report Access Report

Business is requesting a report that have the following columns details:

User username
Report Name Period Inventory Valuation Report
Source Of Report Scheduled Process or OTBI

The purpose of the business is control and audit user access.
We saw some standard reports, but are more granular and have the duties, and job roles details.
We are looking for only the report access for each active user.
