After 11g Forms Upgrade: Java Security Warning After Login/Causes Freezing if Login not Successful

After the OFM 11g forms upgrade, is a security warning is displayed on the first successful login; however, this warning message causes problems if a user clicks cancel or enters invalid authentication credentials this first time.  When a user successfully logs into the application the first time, a security warning is displayed:  “The application’s digital signature cannot be verified. Do you want to run the application?” The user has the option to check a box to always trust content from this publisher, which is "UNKNOWN."  If the user is not successful on the first login, or hits cancel, the screen freezes and the java process must be killed
