How do we enable the Action radio buttons for Manage Approvals for iProc reqs?

R12:  During req creation in iProc, the user clicks on Manage Approvals button.  On the Checkout: Manage Approvals form, only Insert Viewer radio button is active.  How do we enable the other buttons?
