Login page issue -

Hi experts,   We have EBS R12.1.3 , all of sudden form stopped working & on bouncing the application tier services now the login page not coming....

In Apache error log getting the following the messages--


[Tue Sep 17 12:22:52 2013] [error] [client 192.168.1.208] [ecid: 1379400772:192.168.1.100:16854:0:86,0] mod_oc4j: Failed to find a failover oc4j process for session request for destination: application://oacore (no island or jgroup).
