Special Information in requisition dropping off for eye information

Hi

We are users of Oracle 11.5.10.2

In our iProcurement requisition and purchasing, we created information template for our requisition.  In particular an eye information.

Please see attached.  When it reached the PO desk the special information such as:

Name of staff

Preferred branch

Phone number

Details got dropped off from the list.

Do you know what is the issue here?
