Database Metric "Failed Logins" exceeds threshold, but no Alert or Incident is raised ..

OEM12c (rel. 12.1.0.5), Database 12c (12.1.0.2).  Database Metric "Failed Logins" exceeds threshold, but no Alert or Incident is raised.  Other Metrics such as "tablespace usage exceeds limit" are raised for this Database. Not seeing much useful info, in the gcagent.log.  Please post any suggestions.
