CAC Login Issues for Oracle Forms 12

We implemented CAC-card login about a year ago. It is working fine for everyone except for 1 user. Normally it asks for credentials for OFS and then a 2nd one for OIAM-OUD and finally we get a separate window with the Forms app. But for this user, after the 2nd Cert, he gets a message "This webpage caused Internet Explorer to close and reopen the tab" and then it puts him in a loop of asking for the 2 Certs again. He had been previously been able to login and was using the system but then about a month ago
