Managers receives getRegionCodes Exception :null error while opening change assignment notification

Hasanyone ever seen this error message below and if so do you know how to fix it? Please advise.

getRegionCodes Exception :null

HR Tem in our company make changes to the employee HR information using HR User transactions such as Change Pay, Transfer, Worker Status Change.  After submission the transaction goes through approvals/FYI notification.  Most of these transactions complete successfully.  In a small number of cases, when the Approver opens the Notification they see this error.  The changes in the transaction are not displayed in the notification and Transaction in Moniter search remains Active status. If I query the tables, I see all the changes submitted in the transaction.  Even with this error, if the approver approves the transaction, it records the
