Formula to Show On Hand + On Order - Qty on Sales Orders / Amount of Product Sold Per Month

During implementation, NetSuite built a report to show Overstock Items (1 Year of Supply Based on Last 12 Month Sales). Below is the criteria and results tabs. The report is returning incorrect data and I am not skilled enough in these type of formulas to spot what to correct. Anyone SQL gurus out there see what could be the issue?
