American Express GL/KR (FOR KR USA, CANADA & GDC only) Feed Implementation Form?

Hello experts,

Need help here. Has anyone interacted with this feed implementation form? Need to answer items F through K on this form. Where can I get this information? Is it coming from me as the consultant or should I get a technical resource involved?
