Why can I not give access to my employees to create their own login?

I am trying to give an employee access to view our netsuite site but after filling out the "access" portal on the Employee page and pressing save it brings me to a "New User Reuest" page that will not allow me to submit. when i press submit on the request page it says "Please enter values for Subsidiary."

What am i doing wrong??
