Print "Item Fulfillment" - Packing Slip is missing Item information

It suddenly happened today, when we click print button in "Item Fulfillment", the PDF printout is missing item information. It shows the blank body no item information.

In Customization->Form->Transaction PDF Layout, we have the preferred layout configured years ago, no change for years. the Column field is there on the payout.

This is urgent since we can not print Item Fulfillment any more. Can any one help?
