Why is the Transition in my Workflow not executing?

I am trying to build a workflow that will update the Lead - Marketing Qualified status to Lead - Sales Qualified after the Sales Team has marked two specific Tasks complete.

I have reviewed the existing question: "Update Customer Record / Trigger Customer Workflow when Activity or Email created" and tried to leverage some of the components.

When the Lead is created my workflow is creating the two required Tasks: Schedule Phone Call and Schedule Virtual Meeting.

After the Sales Team marks both Tasks "Completed", I need the Lead Status to change to Lead-Sales Qualified. I am attempting this through a "Transition"
