How to make the HTML Code for a Non-Customer Custom Login Page to work?

Hello


I used the code mentioned in help centre for non-customer login, as it is without changing any thing


And this I add to images and user the link below

if I used my credential to login as employee it shows me the following error


What I am looking for to use the following login page with my company logo
