Unable to change header information on Adobe Acrobat Pro

Hello.

Our purchase order template has our company logo, followed by company name, address, phone number and email address. This information is located in the upper left hand area on the form. We need to change the email address, but can't find the screen to change it.

Any assistance will be appreciated. Thank you!
