Script not Triggering - UserEventScript AfterSubmit to create new record.

Hi there, having a little trouble with the attached script. I had an error before saying that it couldn't find the script file but now that I've corrected that I can't seem to get it to trigger at all. I'm sure the inner workings of it may be completely off, I'm just trying to get a response to begin with. :')

It should happen whenever a new item receipt is saved.

Would appreciate a quick glance at my settings and event type to see what I'm missing.
