Is there a way to put a logo inside the form?

One subsidary manages two product labels, each one requries a diferent SO, EST, PO, etc with their respective label logo. That's done. However, in order to reduce the chance to do an error I want to put the label logo in the form. I've been looking for a way to do it but I didn't have success on that.

I'd appreciate your help, thank you for your time and attention.
