User Access Reset Not Unlocking an Account

We have experienced a few issues where one of our support team attempted to unlock a user account that has been locked out due to too many attempts. I personally have not seen this occur so don't have any specific information. However, the support person is extremely savvy so I'm confident she's doing it correctly. THere's very little to 'do wrong' that would keep the account from unlocking.
