How to add Email and Phone # to Packing Slip

I need to add the customer's phone number and email address to the packing slip template but I can't find a way to do this. Can anyone point me in the right direction?

We are temporarily using a new 3rd party logistics provider for shipping and they won't have access to Netsuite for a bit so I need another method of providing them with the pick/pack and ship details. My plan was to email the packing slip to them.

Any help is appreciated. Thank you.
