Online Customer Form Requiring Password

We have an online customer form that collects basic information for people who are interested in getting special pricing, but who don't want an account set up.

We don't have the form set up to allow access or collect password information, but everytime we submit it it throws an error:

Notice:  Please enter values for Password, Confirm Password.

How do we stop this from happening?
