Customer Survey Form

Hi,

I am struggling creating a survey form for our customers. I have tried to follow the help tutorial and designed the online form template which looks good to me. For some reason when I create the actual form in: Setup -> Marketing -> Online Customer Forms, I cannot get it to work.

It's asking for First name and Last night to be added? I do not require these fields, however I have added them on.

I have a custom record type 'Age Group' and 1 field '18-24' which is a checkbox (going to create more record types and more options), but this is not appearing on the form? The option is not visible?
