Inventory Adjustment Worksheet - Broken

Has anyone else got a problem loading the Inventory Adjustment Worksheet? Every time I try to load it it takes several minutes and then ends up at the "unexpected error has occurred" page. I have tried it on more than one user account and on mac and pc with the same results.

Find it under Transctions > Inventory > Adjust Inventory Worksheet
